In between cleaning/exams, receive a free touch up polish to keep you teeth feeling fresh and clean. ** Offer only available to existing patients after each completed cleaning/exam.

Cancellation Policy

To cancel or reschedule, please contact us 24 hours before your scheduled appointment. All appointment changes within 24 hours are assessed a $25 rescheduling fee per patient.

Our office is located on Waukegan Road one block south of Lake Avenue.  Ample parking is available in the private lot behind our building on Maplewood Lane.
